Measure straight across the widest points for width, then head to foot for length.

Allow for real-world variation
Mattress and textile dimensions can have small manufacturing tolerances. Measure the mattress you own and do not rely on the bed-frame label alone.
Near the 40 cm limit?
If the total mattress-and-topper depth is close to 40 cm, also consider edge shape and how much fabric is needed to tuck beneath the mattress.
